Prescription drugs in Sweden are covered by a separate high-cost protection system (lakemedelsformanerna) that caps annual out-of-pocket costs at 2,850 kr over a rolling 12-month period. The system uses a graduated subsidy structure: the patient pays the full cost for the first 1,425 kr, then 50% up to 2,850 kr cumulative patient cost, 25% up to 4,275 kr total cost, 10% up to 5,700 kr total cost, and nothing after the 2,850 kr patient cap is reached.
The Dental and Pharmaceutical Benefits Agency (TLV) determines which drugs are included in the reimbursement system based on cost-effectiveness analysis. Generic substitution is mandatory at pharmacies unless the prescribing doctor explicitly objects or the patient is willing to pay the full price difference. Sweden has a well-functioning generic drug market, with generic substitution saving the system approximately 3 billion kr annually.
For children under 18, all prescription drugs included in the benefits system are completely free of charge. This ensures that families do not face financial barriers to essential medications for their children. Over-the-counter medications are available at pharmacies and, since 2009, at other retailers such as grocery stores for a limited range of products including paracetamol and nicotine replacement therapy.
