Wait times are one of the most significant challenges facing Canada's healthcare system. The median wait time from GP referral to specialist treatment was approximately 27.7 weeks in 2023, nearly double what it was two decades ago. Wait times vary dramatically by province and specialty, with orthopedic surgery and ophthalmology often having the longest waits.
Emergency department overcrowding is another persistent issue, with average wait times of 3–4 hours in many urban centres, and some patients waiting 8–12 hours or more during peak periods. Rural and northern communities face additional challenges due to limited specialist availability, often requiring patients to travel significant distances for care.
The federal and provincial governments have invested billions in wait-time reduction strategies, including surgical backlogs recovery programmes, expanded use of virtual care, and team-based primary care models. Despite these efforts, Canada consistently ranks near the bottom among OECD countries for healthcare wait times, driving some patients to seek care in the United States or through private clinics.
