India is often called the 'pharmacy of the world,' producing approximately 20% of the world's generic medications by volume. This massive generic pharmaceutical industry translates into some of the lowest drug prices globally. The government regulates prices of essential medications through the National Pharmaceutical Pricing Authority (NPPA), which caps prices on over 850 medicines listed in the National List of Essential Medicines.

Common medication costs are remarkably affordable: a monthly supply of metformin (diabetes) costs approximately ₹30-100 ($0.35-1.20), atorvastatin (cholesterol) costs ₹50-200 ($0.60-2.40), and amlodipine (blood pressure) costs ₹20-80 ($0.25-1.00). Even newer specialty medications are typically 50-90% cheaper than in the US due to India's compulsory licensing provisions and patent laws. Prescription regulations in India are less strictly enforced than in Western countries, with many medications available over the counter at pharmacies despite technically requiring prescriptions. Online pharmacy platforms like PharmEasy, Netmeds, and 1mg have grown rapidly, offering additional discounts of 15-25% and home delivery services. However, counterfeit and substandard medications remain a concern, particularly from unlicensed sellers, with the WHO estimating that approximately 10-20% of medicines in developing markets may be substandard or falsified.
