Maternity care in Germany is comprehensively covered by the GKV system at no additional cost to the patient, exempt from all copayments. Coverage includes all prenatal checkups (approximately 10-12 during pregnancy), three standard ultrasound scans, blood tests, genetic screening if indicated, hospital delivery, midwife care, and postnatal follow-up for 8 weeks after birth. Midwife home visits during and after pregnancy are fully covered and available to all insured women.

Germany has a strong tradition of midwife-led care alongside obstetrician care. Every pregnant woman is entitled to a midwife (Hebamme) for prenatal support, birth assistance, and postnatal home visits, with costs fully covered by insurance. Hospital births account for approximately 98% of deliveries, with birthing centres (Geburtshausar) and home births also covered by insurance. The caesarean section rate is approximately 30%, with both planned and emergency caesareans fully covered.

Maternity leave provisions are among the most generous globally. Mutterschutz (maternity protection) provides 6 weeks of pre-birth and 8 weeks of post-birth leave at full salary, followed by up to 14 months of Elterngeld (parental allowance) at 65-67% of net salary (capped at €1,800 per month). Additional prenatal services such as specialized ultrasounds, prenatal yoga, or birth preparation courses beyond the standard coverage may be offered by some GKV insurers as bonus benefits.
