Mexico's public healthcare system operates through several institutions: IMSS (Instituto Mexicano del Seguro Social) covers approximately 65 million formal-sector workers and their families, ISSSTE (Instituto de Seguridad y Servicios Sociales de los Trabajadores del Estado) covers approximately 13 million government employees and families, and IMSS-Bienestar (formerly INSABI) aims to provide coverage for the approximately 50 million uninsured Mexicans. Together, these institutions provide healthcare to the majority of the population.
IMSS and ISSSTE provide comprehensive coverage including GP visits, specialist consultations, hospitalization, surgery, maternity care, and prescription drugs at no additional cost to enrolled members. Employees contribute approximately 2.375% of their salary to IMSS, with employers adding roughly 7-8% and the government contributing a further amount. ISSSTE contributions are approximately 12.75% of salary split between employee and employer. Care is delivered through a network of family medicine clinics, general hospitals, and specialty centres.
Despite universal coverage goals, significant challenges remain. IMSS facilities often face overcrowding, long waiting times, and supply shortages. Waiting times for specialist appointments can range from 2-8 weeks, and surgery waiting lists can extend several months. Quality of care varies significantly by region and facility, with rural areas particularly underserved. These limitations drive many Mexicans who can afford it to seek care in the private sector.
