Each method has distinct CPT codes, and the fee depends on the lesion's size and body location. When the lesion is removed for medical concern, the tissue is sent to pathology and the pathology lab generates a separate charge for interpretation. Insurance coverage hinges entirely on whether the removal is medically indicated (suspicious appearance, irritation, bleeding) or cosmetic — the latter is generally not covered and is billed as self-pay.
