Prescription drugs in the Netherlands are covered under the basic health insurance package, subject to the annual deductible of €385. The Dutch government maintains a comprehensive drug reimbursement system (Geneesmiddelenvergoedingssysteem, GVS) that groups therapeutically equivalent medications together and sets a maximum reimbursement price for each group. If a patient chooses a more expensive medication within the group, they pay the difference out of pocket.

Generic prescribing is strongly encouraged in the Netherlands, with approximately 75% of all prescriptions filled with generic medications. Pharmacists are allowed and encouraged to substitute generics unless the prescribing physician specifically indicates otherwise. The preference policy (preferentiebeleid) allows insurers to designate a preferred brand for each active ingredient, further driving down costs. Patients who require a non-preferred brand may face additional charges.

Over-the-counter medications are not covered by insurance and must be purchased at pharmacies or drugstores. Common OTC medications like paracetamol and ibuprofen are relatively affordable at €2-8 per package. The Netherlands has one of the lower per-capita prescription drug expenditures in Western Europe, partly due to aggressive generic substitution policies and effective price negotiations.
