Mental healthcare in Germany is comprehensively covered by both GKV and PKV insurance systems. Outpatient psychotherapy is a statutory benefit, with patients entitled to cognitive behavioural therapy (CBT), psychodynamic therapy, analytical psychotherapy, and since 2020, systemic therapy. Psychiatric consultations, inpatient psychiatric care, and prescription psychiatric medications are all covered with standard copayments. There are no session limits during the approved treatment period.
The primary challenge with mental healthcare in Germany is access. Waiting times for outpatient psychotherapy average 3-6 months, with some regions experiencing waits of 9-12 months or more. Since 2017, a new appointment service (Terminservicestelle) guarantees an initial consultation with a psychotherapist within 2-4 weeks, but this does not guarantee ongoing treatment. Approximately 27% of the German adult population experiences a mental health disorder in any given year, but only about 19% of those affected receive treatment.
Private psychotherapy outside the insurance system costs €80-150 per 50-minute session. The Psychotherapeutenausbildungsreformgesetz (psychotherapy training reform) of 2020 is gradually increasing the number of licensed psychotherapists. Germany has approximately 48,000 licensed psychotherapists, but their distribution is heavily skewed toward urban areas, leaving rural regions significantly underserved. PKV patients generally face shorter waiting times as many therapists prefer treating privately insured patients due to higher reimbursement rates.
