Denmark uses a unique tiered reimbursement system (medicintilskud) for prescription drugs based on individual annual expenditure. The system operates on a sliding scale: no reimbursement for the first 1,075 kr spent annually, 50% reimbursement for spending between 1,075-1,820 kr, 75% for spending between 1,820-3,300 kr, and 85% for all spending above 3,300 kr. For chronically ill patients with very high drug costs, a safety net ensures costs do not exceed approximately 4,650 kr per year.

The Danish Medicines Agency (Laegemiddelstyrelsen) determines which medications qualify for reimbursement and negotiates prices with manufacturers. Generic substitution is standard practice, with pharmacists required to offer the cheapest available generic alternative. Denmark has one of the highest generic penetration rates in Europe, with generics accounting for approximately 80% of all prescriptions by volume.

Children under 18 benefit from the same reimbursement tiers but applied to the household's total spending rather than individually. For those with the lowest incomes, municipalities can provide additional assistance (kronikertilskud or enkeltilskud) for essential medications. Terminal patients receive all prescription drugs free of charge. Over-the-counter medications are available at pharmacies, and some products like paracetamol can be purchased at supermarkets and convenience stores.
