Japan's Long-Term Care Insurance system (Kaigo Hoken), introduced in 2000, provides comprehensive care for elderly residents. All residents aged 40 and above pay mandatory premiums, and those aged 65+ (or 40-64 with specific age-related conditions) can access services after a needs assessment. Care levels range from Support Level 1-2 (light assistance) to Care Level 1-5 (increasing dependence), with each level determining the monthly service allowance.

Copayment rates for long-term care services are income-based: 10% for most users, 20% for higher-income individuals, and 30% for the highest earners (introduced in 2018). Monthly service caps range from approximately ¥50,320 for Support Level 1 to ¥362,170 for Care Level 5. Services include home-help visits, day care, short-stay respite care, equipment rental, home modifications, and institutional care in special nursing homes (tokubetsu yogo rojin homu).

Despite the comprehensive system, Japan faces severe challenges with its aging population. With over 29% of the population aged 65+, demand for care services continues to outpace supply. There is a critical shortage of care workers, with approximately 690,000 additional workers needed by 2040. Monthly costs for institutional care (special nursing homes) average ¥50,000-130,000 out of pocket depending on income and care level, while private elderly homes cost ¥150,000-500,000+ per month.
