Singapore's healthcare financing operates on the principle of shared responsibility through the '3M' system. Medisave is a mandatory medical savings account into which working Singaporeans and permanent residents contribute 8-10.5% of their monthly wages (depending on age). These funds can be used for hospitalization expenses, certain outpatient treatments, and health insurance premiums. The Medisave contribution cap is S$68,500 (Basic Healthcare Sum).

MediShield Life is a mandatory national health insurance scheme providing coverage for large hospital bills and selected costly outpatient treatments such as dialysis and chemotherapy. Premiums are age-adjusted, ranging from approximately S$130 per year for those aged 21-30 to S$2,400+ per year for those over 80. MediShield Life premiums can be paid from Medisave. The scheme covers treatment in B2 and C class wards in public hospitals with deductibles of S$1,500-3,000 per year and co-insurance of 3-10%.

Medifund is the safety net for those who cannot afford medical bills even after Medisave and MediShield Life. It is a government endowment fund providing assistance to needy Singaporeans on a case-by-case basis through hospital-based Medifund committees. Additionally, many Singaporeans purchase Integrated Shield Plans (ISPs) from private insurers, which provide coverage above MediShield Life for higher ward classes (A or B1 wards) and private hospital treatment, with monthly premiums of S$20-200+ depending on coverage level and age.
