Germany's long-term care insurance (Pflegeversicherung), introduced in 1995, is a mandatory fifth pillar of the social insurance system. All GKV and PKV members are automatically covered. Contributions are 3.4% of gross income (split between employer and employee), with childless individuals over 23 paying an additional 0.6%. Care levels (Pflegegrade 1-5) are determined by a standardized assessment, with benefits increasing by care level.

Monthly benefits for home care range from €125 (Pflegegrad 1 service amount) to €2,095 (Pflegegrad 5) for professional home care services, or €332 (Pflegegrad 2) to €947 (Pflegegrad 5) as a cash benefit for informal/family carers. Residential nursing home benefits range from €770 (Pflegegrad 2) to €2,005 (Pflegegrad 5). However, these amounts rarely cover the full cost of care, particularly in nursing homes where the average resident's out-of-pocket contribution is approximately €2,400-2,800 per month for accommodation, meals, investment costs, and the uncovered care portion.

Germany faces a growing care crisis driven by demographic aging and a severe shortage of care workers. Approximately 5 million people currently receive long-term care benefits, a number projected to rise to 6-7 million by 2040. The care worker shortage is estimated at 200,000 positions, leading to recruitment from Eastern Europe and the Philippines.
