Long-term elder care in Switzerland is financed through a combination of mandatory health insurance, federal/cantonal old-age insurance (AHV), supplementary benefits (Erganzungsleistungen), and substantial out-of-pocket payments by residents and their families. Nursing care costs in care homes are split three ways: health insurance covers a fixed daily amount based on care intensity (CHF 9-108 per day), the resident pays up to CHF 23 per day for nursing care, and the remaining nursing costs are covered by the canton or municipality.
The total monthly cost of a nursing home (Alters- und Pflegeheim) in Switzerland ranges from CHF 6,000-12,000+ depending on the canton, level of care, and facility quality. This includes accommodation, meals, hotel services, and the resident's share of nursing costs. Zurich, Geneva, and Basel tend to have the highest costs, while less urban cantons may be somewhat more affordable. Supplementary benefits (EL) are available to residents whose AHV pension and assets are insufficient to cover care costs.
Home-based care (Spitex) is increasingly promoted as an alternative to institutional care and is partially covered by basic health insurance. Insurance covers prescribed nursing services at fixed rates, while the patient pays a contribution of up to CHF 15.95 per day. Household help and non-medical support through Spitex typically cost CHF 30-70 per hour and are not covered by health insurance. Switzerland has approximately 1,600 care homes housing around 160,000 residents. With 18.7% of the population aged 65+, demand for elder care services continues to grow.
